妊娠B级是指动物生殖实验中并未显示对胎儿的危险性，但尚无妊娠妇女的对照研究；或在动物生殖实验显示出生育力下降以外的不良影响，但在对妊娠早期妇女的对照试验中并不能证实该不良影响（且无该药在妊娠中晚期造成危险的证据）。

妊娠C级是指动物实验证实对胎儿有不良影响（致畸或使胚胎致死或其他），但尚无女性对照研究，或缺乏女性或动物研究可用。本类药物只有在权衡对妊娠妇女的益处大于对胎儿的危害之后，方可使用。

对乙酰氨基酚用于感冒发热、关节痛、神经痛及偏头痛、癌性痛及手术后止痛。

那么，妊娠、哺乳期患者可以使用吗？

妊娠期能用吗？

▍FDA等级：B/C级

对乙酰氨基酚可穿过胎盘[1]。

根据流行病学数据，母亲在孕期使用对乙酰氨基酚后，没有观察到胎ㄦ重大先天畸形的风险增加。尽管隐睾症未被视为主要的出生缺陷，但已观察到母体使用对乙酰氨基酚与隐睾症（睾丸未降）之间的关联[2-5]。

孕期使用正常剂量的对乙酰氨基酚不增加流产或死产的风险；但是，如果用药过量且治疗延迟，胎儿死亡或自然流产发生的几率可能会增加[6-8]。

在妊娠晚期[9]孕妇使用对乙酰氨基酚后的病例报告中，报道了产前动脉导管狭窄；尽管这一关联在一项大型观察性研究[10]中未得到证实。

经评估，对乙酰氨基酚可用于治疗早产儿持续动脉导管未闭[11]在多个研究中评估了其他不良事件，如儿童早期的喘息和哮喘，以及子宫内对乙酰氨基酚暴露后对神经发育的不良影响，例如ADHD；结局相关性及二者的因果关系尚无定论[12-15]。

值得指出的是，母亲发烧也与胎儿不良结局有关，包括神经管缺陷、唇腭裂和先天性心脏缺陷。使用解热药治疗母亲发热可以降低以上风险。由于妊娠引起的生理变化，对乙酰氨基酚的某些药代动力学特性可能发生改变。不建议进行剂量调整[16]。

对乙酰氨基酚被认为适于治疗妊娠期疼痛和发热。对乙酰氨基酚可作为剖宫产术后疼痛缓解的多模式治疗方案的一部分[17]，也可用于治疗妊娠期急性偏头痛[18-19]，并被推荐用于治疗诊断为流感并发热的孕妇[20]。为了有效治疗母亲且保护胎儿健康，建议使用时对乙酰氨基酚应选择最低有效剂量和最短有效疗程[21]。

哺乳期能用吗？

对乙酰氨基酚是哺乳期母亲镇痛和退热的好选择。乳汁中的含量远少于通常给婴儿服用的剂量。母乳喂养婴儿的不良反应似乎很少。
